Nurse and CNA  Referral Program Terms and Conditions

1. Referrals  must be licensed as a Registered Nurse (RN), Licensed Practical Nurse   (LPN),  Licensed Vocational Nurse (LVN), or a Certified Nursing Assistant in the United States. 
2. The referral bonus for an RN is up to $750,  the referral bonus for an LPN is  up to $600 and up to $300 for a CNA. See #5 for details about how bonus payments are calculated.
3. Referrals must be submitted via the online form below OR via communication directly  between you and a Parkway Healthcare Talent Acquisition Specialist or the Director.
4. We will contact your referral in order to introduce them to Parkway Healthcare  Staffing and talk about open positions. Your referral is under no obligation to work with us. If they decide to work with us, you will  receive official correspondence from Parkway Healthcare Staffing  confirming your eligibility to earn the referral bonus.
5. Upon  becoming eligible to earn the referral bonus, your referral must work a  minimum number of hours with Parkway Healthcare Staffing in order for you  to qualify for the bonus payment. For RNs: At 100 hours, you earn $250. At  200 hours, you earn another $250, at 300 hours you earn another $250 for a   total of $750.  For LPNs: At 100 hours, you earn $200. At 200 hours,  you earn another $200, at 300 hours you earn another $200 for a total of $600.  For CNAs:  At 100 hours, you earn $100. At 200 hours,  you earn another $100, at 300 hours you earn another $100 for a total of $300. Hours worked must be completed within one (1) year of the date of your  referral submission to us. 
6. If you qualify for a bonus and you are not an employee working on an assignment  with us, you will be contacted using the information provided on the  referral form to confirm your address before any bonus payment is mailed. 
7. Parkway  Healthcare Staffing reserves the right to cancel the Referral Bonus Program for CNAs and Nurses at any time, for any reason.
8. No  purchase is required.
9. You must be an individual person (i.e., not a business entity) and a legal resident of the United States who is at least 18 years of age to participate in the Program. The Referral Bonus Program is not valid outside of the United States.
